Purpose

This policy explains how our practice supports clients who wish to submit massage therapy services for Flexible Spending Account (FSA) or Health Savings Account (HSA) reimbursement. Our role is to provide accurate, professional documentation; reimbursement decisions are made solely by the client’s plan administrator.


Provider Credentials

All services are provided by a state‑licensed massage therapist in good standing. License information is included on all invoices and receipts upon request.


Documentation We Provide

For eligible services, we provide itemized invoices/receipts that typically include:


  • Provider name and credentials (LMT)

  • License number

  • Business name and contact information

  • Client name

  • Date(s) of service

  • Type of service provided (e.g., Therapeutic Massage, Medical Massage)

  • Duration of service

  • Amount paid and payment status

  • Invoice number

  • Statement indicating services were provided by a licensed professional

This documentation is commonly required by FSA/HSA administrators.


Eligible Services

Massage therapy may be considered eligible for FSA/HSA reimbursement when deemed medically necessary by the client’s plan administrator. Eligibility requirements vary by plan.


Common examples that may qualify include services supporting:

  • Musculoskeletal pain or dysfunction

  • Postural strain or repetitive‑use injuries

  • Recovery from injury or surgery (when cleared)

  • Chronic muscle tension affecting daily function

Note: Relaxation‑only services may not be eligible under some plans.

Client Responsibilities

  • Clients are responsible for verifying eligibility with their FSA/HSA plan administrator.

  • Some plans may require a Letter of Medical Necessity (LMN) from a licensed healthcare provider. Obtaining an LMN is the client’s responsibility.

  • Submission timelines and documentation requirements vary by plan.

Important Disclosures

  • We do not guarantee reimbursement.

  • We do not directly bill FSA or HSA administrators.

  • We do not represent ourselves as FSA/HSA‑approved.

  • Reimbursement decisions are made exclusively by the client’s plan administrator.


Payment Policy

All services are private‑pay at the time of service. Clients may submit provided documentation to their FSA/HSA for potential reimbursement according to their plan rules.
